There are no specific proformas, templates or pre-qualification questionnaires to be completed when submitting an expression of interest. It is therefore important that the participant takes due cognizance of the Conditions for Participation noted in the following sub-sections.
III.2)CONDITIONS FOR PARTICIPATION
III.2.1)Personal situation of economic operators, including requirements relating to enrolment on professional or trade registers
Information and formalities necessary for evaluating if requirements are met: (1) All participants will be required to produce a certificate or declaration demonstrating that they are not bankrupt or the subject of an administration order, are not being wound up, have not granted a deed of trust, are not the subject of a petition presented for sequestration of their estate, have not had a receiver, manager or administrator appointed and are not otherwise apparently insolvent;
(2) All participants will be required to produce a certificate or declaration demonstrating that the participant, their directors, or any other person who has powers of representation, decision or control of the participant has not been convicted of conspiracy, corruption, bribery or money laundering;
(3) Details of legal status and organisation profile indicating relationship with any parent company;
(4) Any conflicts of interest must be disclosed;
(5) Details of requirements for Performance Bond, Parent Company Guarantee etc will be set out in the tender documents. A performance bond will be required;
(6) Details and copies of all certificates of membership of all relevant professional or trade bodies must be provided.
III.2.2)Economic and financial capacity
Information and formalities necessary for evaluating if requirements are met: (1) All participants will be required to provide a reference from their bank;
(2) All participants will be required to provide evidence of relevant insurances including employer's liability, public liability and professional indemnity insurance. Minimum level of cover 10 000 000 GBP for employer's and public liability, 5 000 000 GBP for professional indemnity;
(3) All participants will be required to provide a statement covering the 3 previous financial years including the overall turnover of the candidate and the turnover in respect of activities which are of a similar type to the subject matter of this notice.
III.2.3)Technical capacity
Information and formalities necessary for evaluating if requirements are met:
(1) A statement of the candidates average number of managerial staff and labour force over the previous 3 years;
(2) Details of the educational and professional qualifications of their managerial staff who would be responsible for providing services and carrying out the work under the contract. Provide key staff CVs;
(3) A list of works of a similar nature to the subject matter of this notice carried out over the past 5 years, detailing the value of the consideration received, when and where the works were carried out and whether they were carried out according to the rules of the trade or profession and properly completed. Include references from 2 clients on schemes of a similar nature;
(4) Environmental policy;
(5) Organisational structure;
(6) Quality accreditations;
(7) Experience of working under the JCT form of Contract with Contractor's Design 2005 and other forms of Design and Build Contracts;
(8) Design Philosophy;
(9) Experience of working with novated design consultants and collaborative working;
(10) Principal business activity;
(11) Equal opportunities and diversity policy;
(12) A summary of the participants Health & Safety Policy (Please do not submit a full Health & Safety Policy Document);
(13) Health & Safety record over the past 5 years;
(14) Company's resource numbers for the past 3 years split into management, technical, administration & operative;
(15) Details of supply chain management and how it works;
(16) Approach to sustainability in both the design and construction process;
(17) Working under the considerate contractors scheme or other such approved schemes.
Minimum level(s) of standards possibly required
Selection of applicants invited to submit a tender for this project will be dependant on demonstrating (with supporting documentation where appropriate) the following: -.
The applicants ability taking into account their particular skills, efficiency and reliability to provide suitably qualified experienced professional, technical and labour recsources and a methodolgy and approach capable of delivering the requirements of this project.
Details of the management team proposed for this project including career details, relevant experience and qualifications.
A proven track record in delivering similar recent projects to the appropriate quality, on time and within budget. Where possible these should be projects that have been completed by the office / team proposed above.
Previous experience with regard to Health & Safety policy and how this would be implemented on this project.

Justification for the choice of accelerated procedure: Completion of the Wyre Forest Primary School review. Exisitng school site was for a first school and is unsatisfactory for its current use as a primary school.
