Informatii despre garantia de participare pentru fiecrare lot in parte se regasesc in Anexa B – Informatii suplimentare privind loturile. Perioada de valabilitata a garantiei de participare este cel putin egala cu periaoda de valabilitate a ofertei. Garantia de participare se poate constitui prin:

Documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica sau de atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile legale din tara în care candidatul/ ofertantul este rezident ca persoana juridica.
Aviz de functionare emis de Ministerul Sanatatii - Se va prezenta o copie semnata, stampilata si cu mentiunea „conform cu originalul” pe fiecare pagina.
Mentiune1:Autoritatea contractanta va solicita ofertantului clasat pe primul loc, inainte de transmiterea comunicarii privind rezultatul procedurii de atribuire, de a prezenta pentru conformitate certificatul constatator in original/copie legalizata.

Modul de departajare al ofertelor clasate pe primul loc: In cazul in care in urma stabilirii clasamentului final conform criteriului de atribuire, comisia de evaluare constata ca sunt doua sau mai multe oferte cu acelasi pret, autoritatea contractanta va solicita operatorilor economici in cauza, printr-o clarificare transmisa prin SEAP, o noua propunere financiara pe baza careia vor fi departajati. Reofertarea se face doar pentru loturile la care va exista aceasta situatie.
