— declaratie privind neîncadrarea în prevederile art. 181.
Certificate constatatoare privind îndeplinirea obligatiilor exigibile de plata a impopzitelor si taxelor catre stat, inclusiv cele locale, precum si a contributiei pentru asigurarile socile de stat valabile la data depunerii ofertelor. Se solicita confirmarea privind plata taxelor si impozitelor la bugetul general consolidat si bugetul local – formulare tip eliberate de autoritatile competente din tara în care candidatul /ofertantul este rezident.
Toate aceste documente sa ateste plata la zi a cestor datorii.
Persoane juridice/fizice române: certificat constatator emis de oficiul registrului comertului Autorizatie de functionare / altele echivalente.
Persoane juridice/fizice straine: documente care dovedesc o forma de înregistrare / atestare ori apartenenta din punct de vedere profesional.
Persoane juridice/fizice române/straine: Cerinta minima obligatorie: Statutul societatii si certificatul de înregistrare, care sa confirme posibilitatea efectuarii activitatilor cuprinse la codul CAEN/CPV sub care se face achizitia.

Minimum level(s) of standards possibly required Cerinta minima: Bilantul contabil pe anii 2007, 2008 si 2009, vizat si inregistrat de organele competente si/sau, dupa caz rapoarte anuale, scrisori de audit financiar si contabil, precum si orice alte documente legale edificatoare prin care candidatul/ofertantul isi poate dovedi capacitatea economico-financiara.
Cerinta minima: Declaratie privind cifra medie de afaceri pe ultimii 3 ani, in domeniul de activitate aferent obiectului prezentului contract de achizitie publica.
Cerinta minima: Prezentarea informatiilor solicitate cu privire la obligatiile contractuale fata de alti beneficiari / clienti.
Enumerare sau copii dupa contracte in desfasurare fata de alti beneficiari in contracte similare daca exista.

Minimum level(s) of standards possibly required
Cerinta minima: Completarea unui formular – tabel, cuprinzând valori, perioade de livrare, beneficiari/cantitati, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati. Livrarile de produse se confirma prin prezentarea unor certificate/documente emise sau contrasemnate de o autoritate ori de catre clientul beneficiar. In cazul in care beneficiarul este client privat si, din motive obiective, operatorul economic nu are posibilitatea obtinerii unei certificari /confirmari din partea acestuia demonstrearea livrarilor se realizeaza printr-o declaratie a operatorului economic.
Cerinta minima: experienta similara – concretizata prin îndeplinirea unui numar de 3 contracte de furnizare similare în ultimii 3 ani.
Cerinte minime obligatorii:
— Prezentarea de CV-uri, calificari ale personalului ce va fi implicat in desfasurarea contractului,
— CV pentru personalul implicat în asigurarea service-ului (inginer de service autorizat de producator – cu diploma valabila).
Cerinta minima: Autorizarea sa fie valabila, semnata (semnatura autorizata a managerului si stampilata de producator).
Cerinta minima: minim 3 recomandari.
