III.1)CONDITIONS RELATING TO THE CONTRACT
III.1.1)Deposits and guarantees required
Cuantumul darantiei de participare este de 40 000 RON. Perioada de valabilitate a garantiei de participare este egala cu perioada de valabilitate a ofertei (60 zile).

Decontarea prin ordin de plata din bugetul local al Primariei.

III.2)CONDITIONS FOR PARTICIPATION
III.2.1)Personal situation of economic operators, including requirements relating to enrolment on professional or trade registers
Information and formalities necessary for evaluating if requirements are met: Cerinta obligatorie:
— Completarea formularului 12A Declaratie privind eligibilitatea,
— Completarea formularului 12B Declaratie privind neincadrarea in situatiile prevazute la art. 181 din OUG nr. 34/2006.
Cerinta obligatorie:
— Prezentarea certificatului de atestare fiscala privind plata obligatiilor catre bugetul general consolidat de stat eliberat de D.G.F.P. din care sa reiasa faptul ca ofertantul nu are datorii catre Bugetul general consolidat valabil la data deschiderii, în original sau copie legalizata,
— Prezentarea certificatului privind plata obligatiilor catre bugetul local eliberat de catre directia de specialitate din cadrul Primariilor în a caror raza teritoriala activeaza ofertantul din care sa reiasa ca ofertantul nu are obligatii restante la bugetul local valabil la data deschiderii, în original sau copie legalizata În cazul unei asocieri fiecare asociat este obligat sa prezinte cele de mai sus.
Cerinta obligatorie:
— Prezentarea certificatului constatator emis de oficiul registrului comertului în copie conforma cu originalul, semnata si parafata de reprezentantul ofertantului, nu mai vechi de 30 de zile. În cazul unei asocieri fiecare asociat este obligat sa prezinte cele de mai sus,
— Prezentarea de documente care dovedesc o forma de înregistrare / atestare ori apartenenta din punct de vedere profesional din tara de origine a ofertantului traduse legalizat în limba româna În cazul unei asocieri fiecare asociat este obligat sa prezinte cele de mai sus.

Information and formalities necessary for evaluating if requirements are met: Cerinte obligatorii:
— Prezentarea ultimului bilant contabil, si ultima raportare semestriala, vizate si înregistrate de organele competente,
— Cifra medie anuala de afaceri pe ultimii 3 ani: min. 4 000 000 RON.
III.2.3)Technical capacity
Information and formalities necessary for evaluating if requirements are met:
— Cerinta minima obligatore: o lista a lucrarilor executate în ultimii 5 ani insotita de certificari de buna executie pentru cele mai importante lucrari si care vor contine valori, perioada si locul executiei lucrarilor, modul de îndeplinire a obligatiilor, beneficiari indiferent daca acestia sunt autoritati contractante sau clienti privati - Completarea si depunerea formularului 12 F.
Cerinta minima:
Lista de experienta similara: Încheierea si îndeplinirea în ultimii 5 a unor contracte ale caror obiect a fost executia unor lucrari similare cu o valoare cumulata egala sau mai mare decât valoarea minima impusa de 4 000 000 RON.
Lucrari similare: constructii de cladiri industriale si agrootehnice, blocuri de locuinte, cladiri birouri, banci, garaje pentru parcare, centre comerciale, cladiri pentru cinematografe, teatre, sali de concert, hoteluri, hanuri, camine, restaurante, sali de dans, cluburi de noapte, cladiri pentru scoli colegii, universitati, biblioteci, arhive, muzee, edificii religioase, închisori, spitale, sanatorii.
— Declaratie privind echipamentele tehnice, utilaje - Completarea si depunerea formularului 12H,
— Informatii privind personalul tehnic de specialitate si de asigurarea calitatii - Completarea si depunerea formularului 12I,
— Informatii privind subcontractantii - Completarea si depunerea formularului 12G,
— Standarde de asigurarea calitatii - Certificare ISO 9001 privind certificarea sistemului de management al calitatii pentru lucrari de constructii, valabil la data deschiderii ofertelor,
— Standarde de protectia muncii si a mediului - Certificare a sistemului de management al securitatii si sanatatii în munca pentru activitati în domeniul constructii certificate OHSAS 18001, valabil la data deschiderii ofertelor. Certificare a sistemului de management al mediului pentru activitati din domeniul constructii certificat ISO 14001, valabil la data deschiderii ofertelor.
