Garantie pentru participare constituita prin virament bancar sau instrument de garantare emis de o societate bancara ori de o societate de asigurari, in cuantum de 30 000 RON.

Information and formalities necessary for evaluating if requirements are met: — Ofertantul va completa Formularul nr. 12 A declaratie privind eligibilitatea,
— Ofertantul va completa Formularul nr.12B declaratie privind neîncadrarea în situatiile prevazute la art. 181 din Ordonanta de Urgenta a Guvernului nr. 34/2006, cu modificarile si completarile ulterioare,
— Ofertantul va completa Formularul nr.12C declaratie privind calitatea de participant la procedura,
— Ofertantul va prezenta Certificatul constatator de atestare fiscala eliberat de ANAF precum si certificatul de atestare fiscala privind plata taxelor si impozitelor locale,
— Ofertantul va prezenta Certificat constatator emis de oficiul registrului comertului de pe lânga camera de comert si industrie nationala sau teritoriala in termen de valabilitate.

— Ofertantul va completa formularul nr. 12 D din Sectiunea IV “Formulare “ - Declaratie privind lista principalelor livrari de medicamente in ultimii 3 ani. Ofertantul va specifica obligatoriu obiectul contractelor de furnizare(conditia este impusa numai distribuitorilor/comerciantilor),
— Ofertantul va prezenta cel putin 3 recomandari din partea beneficiarilor/clientilor pentru furnizarea de medicamente,
— Ofertantul va completa formularul nr. 15 Sectiunea IV “Formulare Declaratie pe proprie raspundere privind livrarea produselor",
— Ofertantul va prezenta Autorizatia de punere pe piata eliberata de Agentia Nationala a Medicamentului pentru produsul ofertat.
