Garantia de participare: lot 1 = 26 000 RON; lot 2 = 31 000 RON.
Garantie de buna executie: 5 % din valoarea, fara tva, a contractului care urmeaza a fi incheiat.

Bugetul local, lista de investitii aprobata de Primaria Municipiului Bucuresti.

Acord de asociere - original, semnat si stampilat. Acest acord va trebui sa fie legalizat pana la momentul depunerii ofertelor.

Information and formalities necessary for evaluating if requirements are met: 1. Fisa de prezentare (formular 1);
2. Declaratia privind calitatea de participant (formular 2);
3. Declaratia privind eligibilitatea (formular 3);
4. Declaratie privind neîncadrarea în prevederile Art 181 (formular 4);
5. Istoricul litigiilor (formular 5);
6. Certificat de atestare fiscala eliberat de ANAF, din care sa reiasa plata la zi a datoriilor la bugetul consolidat de stat - valabil la data depunerii ofertelor – original sau copie legalizata;
7. Certificat privind plata impozitelor locale din care sa reiasa plata la zi a datoriilor - valabil la data depunerii ofertelor - original sau copie legalizata;
8. Certificatul de înregistrare emis de Oficiul Registrului Comertului;
9. Certificat constatator emis de Oficiul National al Registrului Comertului cu maxim 30 de zile calendaristice înainte de data depunerii ofertei din care sa rezulte codurile CAEN: pentru obiectul contractului de achizitie.

Information and formalities necessary for evaluating if requirements are met: 10. Declaratia pe propria raspundere de incadrare in categoria IMM-urilor (formular 8 cu anexe) - (daca este cazul) – original, semnat, stampilat, valabil la data depunerii ofertei;
11. Formularul de Informatii generale (formular 9) – original, semnat, stampilat, valabil la data depunerii ofertei;
12. Bilanturile contabile pentru anii 2007, 2008 si 2009, vizate si inregistrate de autoritatile competente – copii legalizate.
III.2.3)Technical capacity
Information and formalities necessary for evaluating if requirements are met:
Oferatntul trebuie sa faca dovada:
a) unei experiente similare in obiectul contractului, in valoare de minim 1 000 000 RON, demonstrabil printr-un contract cu obiect similar finalizat in ultimii 3 ani;
b) capacitatii tehnice pentru executarea service-ului in perioada de garantie;
c) capacitatii profesionale pentru executarea service-ului in perioada de garantie;
d) implementarea sistemului de management al calitatii la producatorul echipamentelor;
13. Autorizatia de functionare emisa de Ministerul Sanatatii;
14. Certificat marcaj CE sau Declaratie de conformitate a producatorului prin care sa fie precizata conformitatea produsului cu Directiva europeana in domeniu 93/42/CE;
15. Autorizatia de livrare (formular 6) eliberata de producator din care sa reiasa denumirea autoritatii contractante, denumirea si data procedurii pentru care se elibereaza autorizatia, produsul/produsele pentru care se primeste autorizatie, confirmarea cantitatii solicitate;
16. Autorizarea de la producator (formular 6) a operatorului economic ce va asigura service-ul in perioada de garantie pentru fiecare din echipamentele ce vor fi livrate;
17. Autorizatie emisa de CNCAN pentru operatorul economic ce va asigura service-ul in perioada de garantie, pentru echipamentele ce intra sub incidenta acesteia;
18. Centralizator autorizatii livrare si service (formular 7);
19. Lista contractelor de furnizare, de aceeasi natura cu obiectul achizitiei, pe care le-a executat în ultimii 3 ani (formular 10);
20. Minim trei recomandari din partea beneficiarilor trecuti în formularul 10;
21. Lista personalului autorizat de producator pentru executarea service-ului in perioada de garantie pentru echipamentele ce vor fi livrate;
22. CV-urile personalului (formular 11) ce va asigura service-ul in perioada de garantie;
23. Certificatele de absolvire a cursurilor de pregatire pentru persoanele ce vor asigura service-ul in perioada de garantie;
24. Autorizatiile emise de CNCAN pentru personalul specializat ce va asigura service-ul;
25. Certificatul ISO 9001 – 2000 sau echivalent pentru producator, emis de o autoritate competenta.
