Information and formalities necessary for evaluating if requirements are met: — Declaratie privind eligibilitatea,
— Declaratie privind neîncadrarea în prevederile art. 181 din O.U.G. nr. 34/2006,
— Certificate de atestare fiscala ANAF pentru persoanele juridice privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor catre stat, precum si a contributiei pentru asigurarile sociale de stat la data de 31.7.2010,
— Certificat fiscal privind plata impozitelor si taxelor locale.
Pentru persoane juridice/fizice române:
— Certificat Constatator emis de Oficiul Registrului Comertului de pe lânga Camera de Comert si Industrie Nationala sau teritoriala,
— Certificatul de înmatriculare la Camera de Comert. Pentru persoane juridice/fizice straine: Documente care dovedesc o forma de înregistrare/atestare ori apartenenta din punct de vedere profesional,
— Se vor prezenta toate certificatele si formularele solicitate în Fisa de date a achizitiei si care trebuie sa fie emise de organele similare din tara respectiva.

Information and formalities necessary for evaluating if requirements are met: Fisa de informatii generale:
— Cifra medie anuala de afaceri pe ultimii 3 ani: minim 48 000 RON, respectiv 11.336,80 EUR, pentru 1 euro = 4,2340 lei, la data de 10.8.2010 (curs BNR),
— Bilantul contabil la data de 31.12.2009 vizat de organele competente în domeniu,
— Informatii privind situatia financiara pentru anii 2007, 2008, 2009.
III.2.3)Technical capacity
Information and formalities necessary for evaluating if requirements are met:
— Declaratie privind calitatea de participant la procedura,
— Declaratie privind lista principalelor livrari de produse în ultimii 3 ani,
— Fisa/fise de informatii privind experienta similara: Orice fisa referitoare la un contract încheiat cu un beneficiar/client trebuie sa fie însotita de copia contractului, a actelor aditionale respective si de recomandarea din partea beneficiarului/ clientului respectiv. Se va prezenta: Procesul verbal de receptie a produselor.
Încheierea si îndeplinirea în ultimii 3 ani a cel mult 3 contracte de furnizare de produse similare a caror valoare cumulata sa fie minim 24 000 RON (fara TVA), respectiv 5 668,39 EUR pentru 1 euro = 4,2340 lei la data de 10.8.2010 (curs BNR),
— Autorizatia de furnizare a produselor,
— Declaratie privind efectivul mediu anual al personalului angajat si al cadrelor de conducere,
— Documente emise de organisme acreditate, care confirma certificarea sistemului calitatii ISO 9001/2001 sau echivalent.
