Synopsis:

VA North Texas Health Care System, 4500 S. Lancaster Rd., Dallas, TX 75216 has a requirement for the provision of Home Health Care Services to chronically ill or disabled Veteran beneficiaries. The Contractor shall provide homemaker/home health aide, skilled nurse, physical therapy, occupational therapy, and speech therapy services in the provision of care to the Veteran beneficiary. Services are required in the following counties:

The Contractor shall provide home health care services, to Veteran beneficiaries, at the beneficiary's home.



The Contractor shall provide Skilled Nursing Care, Homemaker Services, Home Health Aide, Physical Therapy, Speech Therapy, and Occupational Therapy to eligible Veteran beneficiaries



The Contractor shall be state and Medicare licensed.



The Contractor and his/her staff shall meet the requirements in the Texas Administrative Code, Title 25, Part 1, Chapter 146 and in VHA Handbook 1140.6.



The estimated total annual quantity, based on current case load, is 217,372 visits.



The period of performance consists of a base year with four one-year option periods which the Government may exercise unilaterally.



The NAICS Code is 621610 and size standard is $13,500,000.



The anticipated solicitation issue date is on or about 2/27/12 with the response due date of 3/20/11.



The Offeror shall possess current applicable state license and Medicare license.



The Offeror shall possess current professional liability insurance.



The Offeror shall register in Central Contractor Registration (CCR) at the website www.ccr.gov then in Online Representations and Certifications Application (ORCA) at the website www.orca.bpn.gov. Veteran-owned businesses shall also register at the website www.vip.vetbiz.gov.



Upon completion of registration in ORCA and VETBIZ if applicable the Offeror shall e-mail his/her facility's DUNS number and complete name, including d/b/a name, to the point of contact for this acquisition identifying the subject of the message as VA-257-12-R-0030.
