Information and formalities necessary for evaluating if requirements are met: O udzielenie zamówienia mog ubiega si Wykonawcy, którzy:
1. speniaj warunki o których mowa w art. 22 ust. 1 ustawy. W celu potwierdzenia tych warunków, Wykonawca ma obowizek zoy:
1) stosowne owiadczenie,
2) wykaz wykonanych, a w przypadku wiadcze okresowych lub cigych równie wykonywanych dostaw - co najmniej jednej dostawy - w okresie ostatnich 3 lat przed upywem terminu skadania ofert, z podaniem: zleceniodawcy, wartoci, przedmiotu oraz dat wykonania a take zaczeniem dokumentów potwierdzajcych, e te dostawy zostay wykonane naleycie. Aby speni ten warunek warto brutto co najmniej jednej wykazanej dostawy nie moe by mniejsza ni 100 000 PLN;
2. nie podlegaj wykluczeniu z postpowania o udzielenie zamówienia na podstawie art. 24 ust. 1 ustawy. W celu potwierdzenia tego warunku Wykonawca ma obowizek zoy:
1) owiadczenie o nie podleganiu wykluczeniu z postpowania o udzielenie zamówienia;
2) aktualny odpis z waciwego rejestru, wystawiony nie wczeniej ni 6 miesicy przed upywem terminu skadania ofert;
3) aktualne zawiadczenie waciwego naczelnika urzdu skarbowego potwierdzajce, e Wykonawca nie zalega z opacaniem podatków lub zawiadczenia, e uzyska przewidziane prawem zwolnienie, odroczenie lub rozoenie na raty zalegych patnoci lub wstrzymanie w caoci wykonania decyzji waciwego organu – wystawione nie wczeniej ni 3 miesice przed upywem terminu skadania ofert;
4) aktualne zawiadczenie waciwego oddziau Zakadu Ubezpiecze Spoecznych lub Kasy Rolniczego Ubezpieczenia Spoecznego, potwierdzajce, e Wykonawca nie zalega z opacaniem skadek na ubezpieczenie zdrowotne i spoeczne, lub potwierdzenia, e uzyska przewidziane prawem zwolnienie, odroczenie lub rozoenie na raty zalegych patnoci lub wstrzymanie w caoci wykonania decyzji waciwego organu – wystawione nie wczeniej ni 3 miesice przed upywem terminu skadania ofert;
5) aktualna informacja z Krajowego Rejestru Karnego w zakresie okrelonym w art. 24 ust. 1 pkt 4-8 ustawy, wystawiona nie wczeniej ni 6 miesicy przed upywem terminu skadania ofert;
6) aktualna informacja z Krajowego Rejestru Karnego, w zakresie okrelonym w art. 24 ust. 1 pkt 9, ustawy, wystawiona nie wczeniej, ni 6 miesicy przed upywem terminu skadania ofert.
