Information and formalities necessary for evaluating if requirements are met: O udzielenie zamówienia mog ubiega si wykonawcy, którzy speniaj warunki, dotyczce:
a) posiadania uprawnie do wykonywania okrelonej dziaalnoci lub czynnoci, jeeli przepisy prawa nakadaj obowizek ich posiadania;
b) posiadania wiedzy i dowiadczenia;
c) dysponowania odpowiednim potencjaem technicznym oraz osobami zdolnymi do wykonania zamówienia;
d) sytuacji ekonomicznej i finansowej.
Oraz nie podlegaj wykluczeniu z postpowania zgodnie z art. 24 Pzp.
Wymagane owiadczenia i dokumenty jakie maj zoy wykonawcy w celu wykazania podstaw do wykluczenia z postpowania o udzielenie zamówienia w okolicznociach o których mowa w art. 24 ust 1 Pzp.
1) Owiadczenie wykonawcy o braku podstaw do wykluczenia – zoone na podst. art. 24 ustawy Pzp – za. nr 3 do SIWZ,
2) Aktualny odpis z waciwego rejestru, jeeli odrbne przepisy wymagaja wpisu do rejestru, w celu wykazania braku podstaw do wykluczenia w oparciu o art. 24 ust. 1 pkt 2 ustawy, wystawiony nie wczeniej ni 6 miesicy przed upywem terminu skadania ofert, a w stosunku do osób fizycznych owiadczenie w zakresie art. 24 ust. 1 pkt 2 ustawy;
3) Aktualne zawiadczenie waciwego naczelnika urzdu skarbowego potwierdzajace, e wykonawca nie zalega z opacaniem podatków, lub zawiadczenie, e uzyska przewidziane prawem zwolnienie, odroczenie lub rozoenie na raty zalegych patnoci lub wstrzymanie w caoci wykonania decyzji waciwego organu — wystawione nie wczeniej ni 3 miesiace przed upywem terminu skadania ofert;
4) Aktualne zawiadczenia waciwego oddziau Zakadu Ubezpiecze Spoecznych lub Kasy Rolniczego Ubezpieczenia Spoecznego potwierdzajace, e wykonawca nie zalega z opacaniem skadek na ubezpieczenia zdrowotne i spoeczne, lub potwierdzenie, e uzyska przewidziane prawem zwolnienie, odroczenie lub rozoenie na raty zalegych patnoci lub wstrzymanie w caoci wykonania decyzji waciwego organu — wystawione nie wczeniej ni 3 miesiace przed upywem terminu skadania ofert;
5) Aktualna informacja z Krajowego Rejestru Karnego w zakresie okrelonym w art. 24 ust. 1 pkt 4—8 ustawy, wystawiona nie wczeniej ni 6 miesicy przed upywem terminu skadania ofert;
6) Aktualna informacja z Krajowego Rejestru Karnego w zakresie okrelonym w art. 24 ust. 1 pkt 9 ustawy, wystawiona nie wczeniej ni 6 miesicy przed upywem terminu skadania ofert;
7) W przypadku gdy Wykonawca ma siedzib lub miejsce zamieszkania poza terytorium RP – Wykonawcy skadaj dokumenty zgodnie z § 2 ust. 2 i § 4 Rozporzdzenia Prezesa Rady Ministrów w sprawie rodzajów dokumentów, jakich moe da zamawiajcy od wykonawcy, oraz form, w jakich te dokumenty mog by skadane z dn. 30.12.2009 r. (Dz.U. Nr 226, poz. 1817).
