THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED.

STATEMENT OF WORK

Medical Distance Education

Description of services
A comprehensive, online staff education program which includes CE/CME Continuing Education, Regulatory Annual Training, Homeland Security courses in addition to a Learning Management System specific courses on the website.

CE/CME Courses
Primary health care professionals within the Facility will receive accredited Health.edu Continuing Education (CE) courses and Physician Continuing Medical Education (CME) courses that are required to maintain state licensure. Courses are offered for 15 disciplines (Professions): Administration, Clinical Laboratory Science, Dietetics, EMS, Medical Records, Nurse Aide/Assistant, Nursing, Occupational Therapy, Pharmacy, Physical Therapy, Physician, Radiology Technology, Respiratory Therapy, Social Work and Support Staff. All accredited courses provided a minimum of one contact hour or more of credit and are available for a minimum of two years from the date released online.

Facility will receive immediate access to the entire online library of courses and will receive an additional 15 to 17 new courses every month. New courses are developed according to healthcare trends, The Joint Commission regulations and topic suggestions from participants. All of the courses are available online.

Participants will test online and receive certificates of completion for every course that may be printed from participants' personal printers.

Regulatory Annual Training Courses
Provide Regulatory Training for clinical and non-clinical staff to help meet the annual training requirements. Courses are provided online for access Facility or any location with Internet access. The courses should be updated annually and may be supplemented with Facility's unique content and procedures via the site specific training portion of the website.

Site Specific Customized Training Functionality
Facility may develop and upload custom courses on the website to train employees online. Facility may upload content without copyright restrictions under 25MB in size in any of the following formats: word processing files, RTF, text, .jpeg, .gif, Windows Media Format, QuickTime, Real Media Player, and Mp3.

Facility may create a test, evaluation, and certificate of completion for each custom course and also generate detailed reports of usage online.

Security and Tracking
Facility has complete access to the usage data accumulated on the website, and participants may access their personal transcripts. Provide a secure login for the Facility's Site Coordinator (the person who implements the program within the facility) as well as Site Delegates (managers who the Site Coordinator authorize access for certain groups of participants).

Program Support
Have experienced Representatives assist Facility with every detail of the program. Provide training to the Site Coordinator and/or Site Delegates on the website functionality, in addition to delivering marketing support to promote the program and providing live customer support for calls from participants.

52.212-1 - INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS (JUN 2008)
52.212-2 - EVALUATION - COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 52.211-6 - BRAND NAME OR EQUAL (AUG 1999)

(a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation.

(b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must--

(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;

(2) Clearly identify the item by--

(i) Brand name, if any; and

(ii) Make or model number;

(3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and

(4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications.

(c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.

(d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation. (End of provision)

52.217-8 - OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days before the contract expires. (End of Clause)

52.217-9 - OPTION TO EXTEND THE TERMS OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days prior to contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 24 months. (End of Clause)
52.232-19 - AVAILABILITY OF FUNDS FOR THE NEXT FISCAL YEAR (APR 1984)
Funds are not presently available for performance under this contract beyond 31 March 2013. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond 31 March 2013, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer.

5352.201-9101 - OMBUDSMAN (APR 2010)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM or AFISRA ombudsmen, [Insert names, addresses, telephone numbers, facsimile numbers, and e-mail addresses]. Concerns, issues, disagreements, and recommendations that cannot be resolved at the MAJCOM/DRU or ARISRA level, may be brought by the concerned party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (703) 588-7004, facsimile number (703) 588-1067.
(d) The ombudsman has no authority to render a decision that binds the agency.
(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. (End of clause)
