III.2) CONDITIONS FOR PARTICIPATION
III.2.1) Personal situation of economic operators, including requirements relating to enrolment on professional or trade registers:
Information and formalities necessary for evaluating if requirements are met: Tenderers are requested to fill in the legal entities form as provided in D.9 of the standard reply form in the tender specifications and provide the documents as required on the form.
III.2.2) Economic and financial capacity:
Information and formalities necessary for evaluating if requirements are met: a) balance sheets or summaries thereof for at least the past 3 financial years for which accounts have been closed, where publication of balance sheets is prescribed under company law in the country in which the tenderer is established. Where the company has been trading for less than 3 years the following documents shall be provided:
€ balance sheets or summaries for the period for which the company has been trading,
€ a statement of the company's cash flow forecast for the current year and a bank letter outlining the current cash and credit facility position shall be provided;
b) statement of the total turnover, and turnover relating to the provision of similar services, realised over a period of the past 3 years, or for the period the company has been trading (if less than 3 years).

1. A comprehensive company profile including:
€ description of activities, including location of the contractor's premises,
€ project management experience, in particular in an European context, including quality assurance procedures,
€ policy and procedures for staff training,
€ staffing levels as well as an organisational chart and CVs of the personnel designated to carry out the services and their roles (including back-ups) and, in particular, that of the person assigned to supervise the contract with Eurofound,
€ description of security procedures and procedures to deal with any lost items or non-delivery,
€ description of customer service facilities.
2. List of references for recent similar contracts undertaken during the last 3 years, with values, dates, names and contact phone numbers of at least 5 clients.
3. Description of the logistical infrastructure and equipment including:
€ transport fleet; quantity and type,
€ description of web-based track and trace facility if any.
In addition for lots 1 and 2 only:
€ description of complete seafreight and airfreight solutions, road transportation management as well as integrated logistics services.
4. Description, if applicable, of any parts of the contract the tenderer intends to subcontract, and the quality control methods for the subcontractor's work.
(Eurofound reserves the right to check the information provided by the tenderer, in particular by visiting the tenderer's premises).

Submission of tenders: tenders must be submitted in triplicate (1 marked € Original' € unbound/unstapled € and 2 copies) in a sealed envelope, which is enclosed in a second sealed envelope. The inner envelope must be marked, 'Invitation to tender No 10/0364/01. For the attention of: Ms Inma Kinsella. Tender from: (name of tenderer). Not to be opened by the mail service. To be opened by the Opening Committee only'. Note: If self-adhesive envelopes are used, they must be sealed with adhesive tape and the sender must sign across this tape. Submission of tenders by facsimile or by electronic mail is not acceptable and renders these tenders inadmissible.
Tenderers are requested not to use plastic covers or plastic binders, staples etc, and not to include unnecessary amounts of paper.
