Information and formalities necessary for evaluating if the requirements are met: — di essere iscritta, alla Camera di Commercio, industria, artigianato e agricoltura o equivalente in paesi dell’UE, indicando inoltre:
a) la natura giuridica;
b) la denominazione;
c) la sede legale;
d) la data inizio attività;
e) l'oggetto attività;
f) i dati anagrafici del titolare o, in caso di società di tutti gli amministratori muniti di potere di rappresentanza;

— che non sussistono le cause di esclusione di cui all'art. 38 del D.Lgs. 163/2006 e s.m.i.,
— di essere in regola con le norme che disciplinano il diritto al lavoro dei disabili (ai sensi dell'art. 17 della legge 12.3.1999, n. 68 "norme per il diritto al lavoro dei disabili"),
— di aver preso visione del progetto della fornitura ed in particolare del capitolato speciale d’appalto intendendolo adeguato ai disposti del D.P.R. 207/2010 e s.m.i. e del documento unico di valutazione dei rischi interferenziali e di assumere l’obbligo di eseguire l’appalto al prezzo proposto nell’offerta e alle condizioni tutte di progetto, avendo valutato nella determinazione del prezzo tutti gli oneri da sostenere per assicurare una regolare e puntuale esecuzione della fornitura.

Information and formalities necessary for evaluating if the requirements are met: Di possedere, con riferimento al triennio antecedente la data di pubblicazione del bando di gara, un fatturato per forniture analoghe a quelle oggetto del presente appalto a favore di committenti pubblici o privati almeno pari a due volte l’importo posto a base di gara di ogni tipologia di fornitura e pertanto almeno 519 800,00 EUR per forniture arredi a disegno(prestazione principale) ed 145 600,00 EUR per forniture arredi di serie (prestazione secondaria).

Di essere in possesso dei requisiti di ordine tecnico-organizzativo di cui all'art. 90 del D.P.R. 207/2010 e s.m.i. in relazione agli importi di lavori richiesti dal presente appalto; ovvero (alternativamente)
Di subappaltare dette lavorazioni a soggetto in possesso di idonea qualificazione ex art. 90 del D.P.R. 207/2010 e s.m.i. (si ricorda che in tal caso l’importo corrispondente a dette lavorazioni pari a 35 200,00 EUR dovrà essere coperto con qualificazione in prestazione principale ai sensi del precedente punto 5) del presente capo e che pertanto l’importo da dichiarare al punto 5), nonchè da comprovare in sede di sorteggio o di aggiudicazione, sarà per le forniture arredi a disegno (prestazione principale) di almeno 555 000,00 EUR anzichè 519 800,00 EUR).
