Cauzione provvisoria pari al 2 % dell'importo dell'appalto costituita con le modalità di cui all'art. 75 del D.Lgs. n. 163/2006.
Impegno di un fideiussore a rilasciare garanzia fideiussoria per l'esecuzione del contratto di cui all'art. 75, comma 8, D.Lgs. 163/2006.
Cauzione definitiva pari al 10 % dell'importo di aggiudicazione da costituirsi con le modalità di cui all'art. 113 del D.Lgs. n. 163/2006.

Information and formalities necessary for evaluating if requirements are met: Dichiarazione sostitutiva, resa ai sensi del D.P.R. 445/2000 e s.m.i., attestante che l'operatore economico non si trova nelle cause di esclusione di cui all'art. 38 del D.Lgs. n. 163/2006 compresa l'indicazione di eventuali condanne per le quali si sia beneficiato ddella non menzione. In caso di ATI o consorzio dette documentazioni dovranno essere prodotto da tutti i soggetti appartenenti al gruppo. Saranno esclusi dalla partecipazione alla gara i soggetti che siano privi dei requisiti di ordine generale (art. 38 D.Lgs. 163/2006 s.m.i.). Requisiti di idoneità professionale: certificazione o dichiarazione sostitutiva resa dal legale rappresentante secondo le modalità previste dal D.P.R. n. 445/2000 o analoga modalità prevista dallo Stato di residenza della ditta, recante l’iscrizione per lo svolgimento di attività inerenti alla gara al Registro imprese o analogo registro dello Stato di residenza, di data non anteriore a 6 mesi con l’indicazione delle persone legittimate a presentare ed impegnare l’impresa con relativa carica sociale. In caso di raggruppamento temporaneo o consorzio, detta documentazione dovrà essere prodotta da tutti i soggetti appartenenti al gruppo.
III.2.2)Economic and financial capacity
Information and formalities necessary for evaluating if requirements are met: a) n. 2 dichiarazioni bancarie di data non anteriore a 6 mesi dalla scadenza del bando, attestante che l’impresa ha sempre rispettato puntualmente i propri impegni. In caso di raggruppamento temporaneo o consorzio dette dichiarazioni devono riferirsi a ciascun soggetto appartenente al gruppo;
b) dichiarazione sostitutiva secondo le modalità previste dal D.P.R. n. 445/2000 o analoga modalità prevista dallo Stato di appartenenza, recante fatturato globale dell’impresa ed importo relativo a forniture rese nel settore oggetto di gara, realizzati negli ultimi 3 esercizi finanziari. In caso di consorzi il requisito economico e finanziario deve essere comprovato unicamente dagli stessi (art. 35 D.Lgs. 163/32006 s.m.i.). In caso di avvalimento si applica l’art. 49 del D.Lgs. 163/2006 s.m.i.

Information and formalities necessary for evaluating if requirements are met:
Elenco principali forniture oggetto del presente appalto regolarmente prestate presso aziende sanitarie/enti/ospedali pubblici nel triennio 2007/2009. In caso di raggruppamento temporaneo di imprese o consorzio ordinario il servizio di importo maggiore deve essere presentato dall’impresa mandataria. In caso consorzi il requisito richiesto deve essere posseduto e comprovato dagli stessi (art. 35 D.Lgs. 163/2006). In caso di avvalimento si rimanda all’art. 49 del D.Lgs. 163/2006 s.m.i.
