Information and formalities necessary for evaluating if the requirements are met: Ai fini della partecipazione i soggetti interessati dovranno produrre, pena esclusione dalla gara, la seguente documentazione:
L) domanda di partecipazione: secondo lo schema fomito dall'ente, che dovrà pervenire, in busta chiusa entro il termine fissato, a pena di esclusione, a mezzo raccomandata per posta o consegnata direttamente a mano, o tramite Agenzie specíalizzate, presso l'ufficio protocollo generale del Comune di Alghero, ad esclusivo rischio del mittente e pertanto non veranno presi in considerazione reclami di sorta nel caso non giunga in tempo utile.
2) modello di autocertificazione, fomito dall'ente, da allegare alla domanda di partecipazione, reso con le forme di cui al D.P.R. 445 del 281121200Q riguardante dichiarazioni che attestano il possesso dei requisiti di cui agli art.li 38, 39, 4I, 42 del D.Lgs. 16312006. Il modello di autocertificazione dovrà essere accompagnato, pena di esclusione dalla gan, da fotocopia del documento di identità, valido, del firmatario. Livelli minimi di capacità richiesti:
1) La raccolta premi complessiva nei rami danni, (escluso il ramo 10 RC Auto) realizzata, nel triennio 20081200912010, non deve essere inferiore 40 000 000,00 EUR.
In caso di raggruppamento d'impresa la ditta capogruppo dovrà dimostrare di possedere per ogni singolo lotto almeno l'80Yo dei livelli minimi di capacità sopra richiesti, fermo restando che il RTI dovranno possedere al l00 % i requisiti richiesti. In caso di consorzio, per i livelli minimi di capacità sopra richiesti, vale quanto specificato dagli artt. 35 e 36 del D.Lgs. 1631200.

Coperture assicurative per enti pubblici: alle ditte partecipanti, è richiesto di aver prestato nel triennio 20081200912010, polizze rami danni in un numero non inferiore a 50 (cinquanta). Saranno ritenute equivalenti le eventuali quote inferiori al I00 % detenute in qualità di coassicuratore e/o componente di RTI.
In caso di raggruppamento d'impresa almeno la ditta capogruppo dovrà dimostrare di possedere il livello minimo di capacità richiesto. In caso di consorzio, per i livelli minimi di capacità sopra richiesti, vale quanto specificato dagli artt.: 35 e 36 del D.Lgs. T63/2006.
