Γ) Δλωση σχετικ με τα μηχανματα τις εγκαταστσεις και τον τεχνικ εξοπλισμ του παρχου.
Δ) Δλωση σχετικ με τα μηχανματα και τον τεχνικ εξοπλισμ που θα διθεσει ο προχος.
Β) Κατλογο των κυριτερων υπηρεσιν που χουν παρασχεθε κατ τα τρα τελευταα τη, που εμφανζεται η αξα,η ημερομηνα παροχς και οι αποδκτες τους, δημσιο ιδιωτικο φορες:

Precise information on deadline(s) for lodging appeals: Εντς 10 ημερν ο ενδιαφερμενος μπορε να λαμβνει με οποιοδποτε τρπο γνση της παρανομς πρξης παρλειψης και μπορε να σκησει πρσφυγη κατ αυτς ενπιον της αναθτουσας αρχς (προδικαστικ πρσφυγη. Η πρσφυγη αυτ αποτελε ρο παραδεκτο για την σκηση ασφαλιστικν μτρων ενπιον του αρμδιου δικαστικο σχηματισμο, χωρς την σκηση της οποας τα ασφαλιστικ μτρα κρνονται απαρδεκτα. Η αναθτουσα αρχ υποχρουται να απντησει αιτιολογημνα εντς 15 ημερν απ την σκηση της πρσφυγης. Εαν παρελθε πρακτη η παραπνω προθεσμα τεκμαρεται η απρριψη της προσφυγς.
