Kopie der Eintragung beifügen (in der Bun-desrepublik Deutschland Handelsregister bzw. Handwerksrolle, bei ausländischen Bietern Unterlagen gemäß Artikel 46 der Richtlinie 2004/18/EG),
— Eigenerklärung zu Eintragungen im Gewerbezentralregister über den Bieter (bei ausländischen Bietern Unterlagen gemäß Artikel 45 der Richtlinie 2004/18/EG).

Information and formalities necessary for evaluating if the requirements are met: — Eigenerklärung über den Gesamtumsatz je Jahr in den letzten 3 abgeschlossenen Geschäftsjahren,
— Eigenerklärung über den Umsatz je Jahr der Waren, die mit der zu vergebenden Leistung vergleichbar ist, in den letzten 3 abgeschlossenen Geschäftsjahren,
— Eigenerklärung: Nachweis von mindestens 2 Referenzen (möglichst unter Angabe der vollständigen Adresse, eines Ansprechpartners und dessen Telefonnummer) bei denen vergleichbare Verträge abgeschlossen wurden. Der Bieter hat die Vergleichbarkeit darzustellen,
— Eigenerklärung über eine ausreichende Betriebshaftpflichtversicherung i.H.v. mind. 1 000 000 EUR je Schadensereignis für Personen-/Sachschäden und mind. 50 000 EUR für Vermögensschäden. Die Gesamtleistung für alle Versicherungsfälle eines Jahres muss das Zweifache dieser Deckungssumme(n) betragen.
Dem gleichgesetzt ist eine Bestätigung des Versicherers, dass im Auftragsfall die Deckungssummen ohne Bedingungen auf die geforderten Summen erhöht werden ODER Nachweis (Kopie) einer entsprechenden Betriebshaftpflichtversicherung mit o.g. Summen.
(Hinweis: Die geforderten Schadensarten und Schadenssummen müssen explizit ausgewiesen sein.).
— Eigenerklärung: Unterauftragnehmer einschl. deren Teilleistungen und Angabe von entsprechenden Referenzen.
Die Vergabestelle behält sich vor, in Zweifelsfragen weitere Eignungsnachweise nachzufordern.
— Eigenerklärung: Allgemeine Informationen über das Unternehmen (z.B. Firmenbroschüre, Zertifikate, Maßnahmen zur Qualitätssicherung),
— Kopie des kompletten Angebotes gemäß Ziffer 3.4.1 der VU (möglichst elektronisch im pdf-Format auf CD).
