Information and formalities necessary for evaluating if the requirements are met: 1) aktueller Nachweis, dass der Bieter im Berufs- oder Handelsregister nach Maßgabe der Rechtsvorschriften des Landes der Gemeinschaft oder des Vertragsstaates des EWR-Abkommens eingetragen ist, in dem er ansässig ist. Hinweis: Unternehmen, die weder im Berufs- noch Handelsregister noch einem anderen Register geführt werden, legen eine Kopie der Gewerbeanmeldung der zuständigen Stelle des Landes, in dem sie ansässig sind (soweit erforderlich) oder einen anderen geeigneten Nachweis (z. B. bereinigter Steuerbescheid) vor, der Aufschluss über die Art der beruflichen Tätigkeit gibt. 2) Eigenerklärung, dass keine rechtskräftigen Verurteilungen der in § 6 EG Abs. 4 VOL/A aufgezählten Straftaten vorliegen (vgl. Information and formalities necessary for evaluating if the requirements are met: 1) Eigenerklärung, dass eine Betriebshaftpflichtversicherung besteht und dass die Versicherung bei Angebotsabgabe nicht gekündigt ist. Die Deckungssumme muss mindestens das 1,5-fache des Auftragswerts betragen. (vgl. Leistungsverzeichnis).
2) Eigenerklärung, dass über das Vermögen des Bewerbers kein Insolvenzverfahren oder ein vergleichbares gesetzliches Verfahren eröffnet oder die Eröffnung beantragt oder dieser Antrag mangels Masse abgelehnt worden ist (vgl. Leistungsverzeichnis).
3) Eigenerklärung, dass sich der Bewerber nicht in Liquidation befindet (vgl. Leistungsverzeichnis).
4) Eigenerklärung, dass der Bewerber seiner Verpflichtung zur Zahlung von Steuern, Abgaben und Beiträgen zur gesetzlichen Sozialversicherung (u.a. auch zur Berufsgenossenschaft) ordnungsgemäß nachgekommen ist (vgl. Leistungsverzeichnis).
5) Eigenerklärung, dass der Bewerber keine Verstöße im Sinne des § 5 des Gesetzes zur Bekämpfung der Schwarzarbeit begangen hat, bzw. keine Eintragungen im Gewerbezentralregister wegen illegaler Beschäftigung bestehen (vgl. Leistungsverzeichnis).
6) Eigenerklärung, in welcher der Bieter bestätigt, dass weder sein Unternehmen, noch Mehrheitsanteilseigner oder Gesellschafter, noch eine Mutter- oder Tochtergesellschaft oder Mitglieder der Bietergemeinschaft auf einer der in den Anlagen zu den Verordnungen 881/2002 und 2580/2001 sowie der Anlage des Standpunktes des Rates 2001/931/GASP befindlichen Terrorlisten erscheint (vgl. 1) Nachweis von mind. einem, max. 3 vergleichbaren Referenzprojekten der letzten drei Jahre durch eine Eigenerklärung als Anlage zum Angebot mit folgendem Aufbau:
— Angabe, ob öffentlicher oder privater Auftraggeber (AG), - Projektbezeichnung und -beschreibung, - Leistungszeit von/bis, - Angabe zum Rechnungswert und Lieferumfang,
— Ansprechpartner des o. g. Auftraggeber mit Namen, E-Mail und Telefonnummer.
Die Kontaktstelle berechtigt, die angegebenen Referenzen selbst auf Richtigkeit zu überprüfen und bei den entsprechenden Ansprechpartner Informationen über das Referenzprojekt einzuholen. Sollten sich dabei Tatsachen ergeben, die den Bieter als nicht geeignet darstellen, kann er vom weiteren Verfahren aufgrund mangelnder Eignung gem. § 19 EG Abs. 5 i. V. m. § 6 EG Abs. 6 lit. e VOL/A ausgeschlossen werden.
2) Eigenerklärung, dass die gemäß Ziff. 3.23 der Vergabe- und Vertragsunterlagen zuzusichernden Qualitätsanfoderungen eingehalten werden. Dies sind insbesondere:
— Produktionsdatum der gelieferten Reifen kleiner 12 Monate,
— Gleichbleibende Qualitäten während der Vertragslaufzeit.
