Information and formalities necessary for evaluating if the requirements are met: Een attest dat uitgereikt werd door de bevoegde overheid en waarin bevestigd wordt dat de inschrijver voldaan heeft aan zijn verplichtingen inzake betaling van de bijdragen voor de sociale zekerheid, tot en met diegene die slaan op het voorlaatste afgelopen kalenderkwartaal vóór de uiterste datum voor de ontvangst van de aanvragen tot deelneming of voor de ontvangst van de offertes, overeenkomstig de Belgische wetgeving of die van het land waar hij gevestigd is. In toepassing van het koninklijk besluit van 20 juli 2005 dient de Belgische inschrijver geen rsz-attest bij zijn offerte te voegen. De aanbestedende overheid zal deze informatie zelf opvragen.

— niet veroordeeld is geweest, bij een vonnis dat in kracht van gewijsde is gegaan, voor deelname aan criminele organisaties, omkoping, fraude of witwassen.

Een geldig ISO 9001 (versie 2008) certificaat, of een verklaring of bewijzen inzake gelijkaardige maatregelen op het gebied van kwaliteitsbewaking of BeSaCC-attest(Belgium Safety Criteria for Contractors).
Een VCA-certificaat (Veiligheid, gezondheid en milieu Checklist Aannemers) ten bewijze dat de firma voldoet aan de minimumeisen op het gebied van veiligheid, gezondheid en milieu of een gelijkwaardig document voor buitenlandse inschrijvers.
Een lijst van gelijkaardige algemene schoonmaakdiensten van minstens 500 000,00 EUR uitgevoerd tijdens de laatste drie jaar, met vermelding van bedrag en datum en van de publiek- of privaatrechtelijke instanties waarvoor zij bestemd waren:
a) indien het diensten aan overheden betreft, worden de diensten aangetoond door certificaten die door de bevoegde overheid zijn opgesteld of goedgekeurd;
b) indien het gaat om diensten aan privaatrechtelijke personen worden de diensten aangetoond door certificaten opgesteld door deze personen of, bij ontstentenis, door een verklaring van de dienstverlener.
Een verklaring die de werktuigen, het materieel en de technische uitrusting vermeldt waarover de dienstverlener zal beschikken voor de uitvoering van de diensten.
