Information and formalities necessary for evaluating if the requirements are met: Une déclaration sur l'honneur, établie conformément au modèle repris en annexe 1 de la circulaire du 21.5.2001 relative à la sélection qualitative des entrepreneurs, des fournisseurs et des prestataires de services (moniteur belge du 18.7.2001, p. 24.532) et attestant que le(s) soussigné(s) ne se trouve(nt) dans aucune des situations visées par les causes d'exclusion reprises à l'article 17 de l'arrêté royal du 8.1.1996.
