Information and formalities necessary for evaluating if the requirements are met: Une déclaration sur l'honneur, établie conformément au modèle repris en annexe 1 de la circulaire du 21.5.2001 relative à la sélection qualitative des entrepreneurs, des fournisseurs et des prestataires de services (moniteur belge du 18.7.2001, p. 24.532) et attestant que le(s) soussigné(s) ne se trouve(nt) dans aucune des situations visées par les causes d'exclusion reprises à l'article 17 de l'arrêté royal du 8.1.1996. L'attention est attirée sur le fait qu'à quelque stade que ce soit de la procédure, le pouvoir adjudicateur peut inviter le(s) soussigné(s) à produire les documents et preuves nécessaires.

Information and formalities necessary for evaluating if the requirements are met: Le soumissionnaire doit satisfaire aux exigences de l'agréation conformément à l'article 3 de la loi du 20.3.1991 organisant l'agréation d'entrepreneurs de travaux, et en fournir la preuve, à savoir:
