Information and formalities necessary for evaluating if the requirements are met: De dienstverlener dient volgende documenten toe te voegen aan zijn offerte:
1. Met uitzondering van de Belgische inschrijvers, voor wie rsz-gegevens langs elektronische weg kunnen bekomen worden, zullen alle inschrijvers bij hun inschrijving een attest voegen, verstrekt door de in hun staat bevoegde instellingen, dat bewijst dat zij voldaan hebben aan hun verplichtingen in verband met de sociale zekerheid van hun werknemers.
2. Fiscale attesten waaruit blijkt dat de Belgische en buitenlandse kandidaat-dienstverlener in orde is met de betaling van zijn belastingen in België respectievelijk het land waar hij gevestigd is (directe belastingen maximum 6 maanden oud; btw-attest maximum 3 maanden oud).
Met zijn inschrijving verklaart de inschrijver zich niet in een toestand van uitsluiting te bevinden, zoals bedoeld in artikel 69 §1 en 2 van het KB van 8.1.1996 betreffende de overheidsopdrachten voor aanneming van werken, leveringen en diensten en de concessies voor openbare werken.

1. De onderzoeken dienen uitgevoerd te worden door een dienstverlener die geaccrediteerd is door BELAC volgens ISO 17025:2005 voor visueel onderzoek met een zelfrijdende camera voor de diameters tussen 200 mm en 900 mm en de codering volgens de norm NBN EN 13508-2:2003 + A1:2011. Het bewijs van accreditatie dient te worden toegevoegd aan de offerte. Indien bij indiening van de inschrijving de accreditatie nog niet werd verkregen, volstaat een kopie van de aanvraag tot accreditatie. Ten laatste op moment van gunning dient de dienstverlener geaccrediteerd te zijn.
Het onderzoek van leidingen met diameter tussen 900 mm en 1 400 mm, eivormige leidingen of gemetselde en betonnen kokers, en de hiervan op te maken rapportering dienen te voldoen aan dezelfde vereisten uit dit bestek, met dezelfde kwaliteit zoals gewaarborgd via de accreditatie. De bekwaamheid om grotere diameters tot 1 400 mm, eivormige leidingen en kokers te onderzoeken en te rapporteren volgens dezelfde eisen en voorwaarden als uit de accreditatie, dient bij inschrijving te worden aangetoond (opgave installatie, opnames van voorgaande onderzoeken, ...).
De inschrijver verbindt er zich toe om, indien hij percelen gegund krijgt, de opdrachtgever in te lichten over eventuele intrekking of stopzetting van zijn accreditatie.
2. De dienstverlener zal slechts evenveel percelen - met een maximum van 2 percelen - toegewezen krijgen in de mate dat hij één volledige installatie voor camera-onderzoek onder accreditatie per perceel ter beschikking heeft. Een volledige installatie bestaat uit 1 voertuig met uitrusting, een inspecteur en voldoende personeel om veilig te werken.
3. Ter beoordeling dient een opgave te gebeuren van de inspectieploegen Elke persoon die een onderzoek beoordeelt, is verplicht een cursus te hebben gevolgd ingericht door VLARIO-OCW en moet geslaagd zijn op de afsluitende interactieve proef. Per opgegeven inspectieploeg moet een bekwaamheidsgetuigschrift worden voorgelegd. De inspecteur dient tevens attesten te kunnen voorleggen van volgende gevolgde opleidingen:
— EHBO o afdalen in riolen.
Andere personen die deel uitmaken van de onderzoeksploeg dienen eveneens de opleiding mbt het afdalen in riolen gevolgd hebben.

4. Ter beoordeling van de rapporteringswijze dient een proefrapportering aangeleverd te worden conform de bestekvoorschriften. Aquafin levert hiertoe beeldmateriaal, een plan met opgave van putten en leidingen en een XML (opvraagbaar bij Aquafin NV, op volgend mailadres ingrid.tersago@aquafin.be). Deze proefrapportering omvat:

— een viewer met de gevraagde functionaliteiten voor de digitale weergave,
— een codering van de vastgestelde schadebeelden conform de norm NBN EN13508-2,
— de bijhorende schriftelijke rapportering op digitale drager,
— het aangevulde XML-bestand conform de norm NBN EN13508-2, de Belgische opmaak voor gegevensuitwisseling (BEFDSS) en de Aquafin-bepalingen.
5. Zowel de contactpersoon als de inspecteur van elke inspectieploeg dienen de Nederlandse taal machtig te zijn.
